APPLY FOR ASSISTANCE** STARTING JANUARY 15th

Application button will be available January 15 at 9 AM.

You will be asked to upload the following documents:

  • Letter from your oncologist (must include the information in the sample)

  • Your most recent taxes or disability benefit letter for 2025.

If you do not file taxes, please let us know.

We are not able to process any applications with missing information.

**To qualify, you must reside in Miami Dade County, be in active treatment for an early staged cancer, or have a stage 4 cancer diagnosis. Active treatment is defined as surgery to remove the cancer, chemotherapy, or radiation. For our purposes reconstruction and immunotherapy are not considered active treatment.​
