Services are available to women in active cancer treatment* that reside in Miami-Dade County. The men's program is available to patients at Miami Cancer Institute.

APPLICATIONS WITHOUT A LETTER WILL NOT BE REVIEWED.

Before you apply, please have your Doctor Letter (see sample) ready to upload. Letter must be on hospital letterhead, signed by the oncologists and include:

1. Diagnosis Date

2. Cancer Type and Subtype (if applicable)

3. Stage (designated by 0-4 or TNM)

Clinical notes or summaries are not accepted.

Applying does not guarantee services.

*Active treatment is considered infusion chemo, radiation, 2 weeks from a cancer removing surgery, or first line treatment of immunotherapy.
